Hi,

Confirm you phone is V5 (PD1612F) or V5s (PD1612DF) first of all, and then try solve problem as follow way:
1.Turn on and then Turn off then phone
2.Select correct models as your phone
3.Factory Reset without data loss (Unlock tab)

p/s: send fastboot command "fastboot oem device-name" to confirm your phone model.

Download & install latest installer or online updates, and then try it again.
p/s: confirm you phone model and thhen select correct model before operation.

Install latest emt installer and then try it again, may some files missing, if the problem not solve contact me with my skype please.

2. uninstall/stop antivirus
3. download new eme setup and install
